A provider-facing SMART-on-FHIR app. Launched from inside an eClinicalWorks patient chart, it surfaces the patient's precomputed HCC opportunities, medication adherence alerts, and quality gaps from the IPA — with supporting chart evidence pulled live from the EMR.

How it launches
  1. 1. Provider opens a patient chart in eClinicalWorks.
  2. 2. Provider clicks the Care Gap Assist app — eCW passes patient + provider context.
  3. 3. Pelica completes the SMART OAuth handshake and maps the patient to the IPA member.
  4. 4. The provider sees gaps, alerts, and chart evidence for that patient.
